In medical tourism, payouts usually live in a spreadsheet someone rebuilds at month end. With the rate defined in the system, the calculation happens with the sale — and nobody argues about the same deal twice.

What the month-end spreadsheet costs

When commission lives in a spreadsheet, three things happen at once: the calculation is rebuilt every month, who earned what gets argued about, and whoever maintains the file becomes irreplaceable.

The real cost is trust. A consultant who cannot see their own number checks it every month, and management answers the same questions every month. Neither side enjoys it.

How rates are defined

Commission is defined per product or service, because margins differ by treatment and a single percentage is never fair across them. Two forms:

  • Percentage — a share of the sale value.
  • Fixed amount — a set figure per case, usually preferred where prices vary a lot.

Rates attach to people, so different consultants can carry different rates on the same treatment. Seniority and individual agreements have somewhere to live.

Payouts attach to the sale

Who earns on a given patient is held on the record. When the sale happens, a commission line is created against the order item — so the payout is calculated at that moment's rate.

That detail matters: changing a rate later does not rewrite past payouts. It is exactly the argument spreadsheet-based teams have most often.

Partners and referrals

Payouts in this sector do not stop at your own team: the referring partner or agency takes a share too. Partnerships are recorded separately, and partner-level tracking runs alongside accounting.

Keeping both sides in one system means "has the patient paid?" and "has the partner been paid?" are answered on the same screen.

Can a consultant see their own commission?
