How to create a web form
Connecting the form on your website to MetoCRM so submissions arrive as leads.

If the contact form on your website is not connected to MetoCRM, submissions land in a mailbox and wait there. The web form integration turns each one straight into a lead — with its source, its campaign and a timestamp.
The connection is made through the form's URL; you do not have to change your website's stack.
Before you start
- The full URL of the page the form lives on.
- A decision on which team will pick up submissions from this form.
Step by step
- Click the Contact Forms button under Leads in the menu on the left.

- On the opened page click the Add button in the upper right corner.

- On the opened page After filling in the fields, click the Save button below.

- Click the Edit button to make changes to the Web Form.

- If you want to delete the Web Form, click on the arrow next to Edit and click the Delete button.

Tips and common mistakes
- Tip Send a test submission from your own site after connecting and confirm it appears in the lead list. A form that is live but untested is the most common cause of silent lead loss.
- Tip If you run separate landing pages per campaign, register each as its own web form — it is the only way to measure which page brings patients.

My form runs on a custom stack. Will it still connect?
In most cases yes. For more involved setups you can use the API and webhooks.
Does a submission become a patient card or a lead?
Your choice: save it directly as a customer, or hold it in the lead list for manual conversion.
How do I keep spam out?
Use a captcha and required-field validation on your form. The cheapest filter is the one that runs before anything reaches MetoCRM.
