How to read the audit log
The audit log shows who changed which record and when: the action, the previous and new values, the user and the request address.

The audit log quietly stores changes made to tracked records. "Who changed this patient's status", "when was that quote deleted", "what did the consultant who left do in their last weeks" — the answers are here.
Each row carries five things: the action (create, update, delete), which record (type and id — patient, quote, consultation, user…), who did it, what changed (previous and new field values) and where from (the address the request came from).
Tips and common mistakes
- Tip A deleted user's trail is preserved: what someone did before leaving does not vanish when their account is removed. That is the detail that earns its keep during staff changes.
- Careful The audit log is closed to the Moderator role — Moderator reaches everything else but not this. The separation is deliberate: an audit trail the audited person cannot read.
- Tip When investigating what a bulk job did, read the audit log alongside the Bulk Operations screen; the two give different detail.
Frequently asked questions
Who can see the audit log?
Admin by default. The Moderator role is deliberately excluded.
Can I restore a deleted patient from the log?
The log shows what changed and when; it is not an undo tool. Deleted records need the separate view and admin help.
Is every change logged?
Creates, updates and deletes on tracked record types are logged. Which types are tracked is defined on the system side.
